Quick Answer
In October 2026, advertised US self-pay prices for obesity GLP-1 medicines run from $149 per month (Wegovy pill 1.5 mg; Foundayo 0.8 mg and, through December 31, 2026, 2.5 mg) to $449 per month for most Zepbound KwikPen maintenance doses. Commercially insured patients whose plan covers the drug can pay as little as $25 per month with a manufacturer savings card (capped). Eligible Medicare Part D beneficiaries pay $50 per month through the Medicare GLP-1 Bridge until December 31, 2027. Several offers end on December 31, 2026.
Deadline watch: the Wegovy new-patient injection offer, the Zepbound and Foundayo savings cards, and the temporary Foundayo 2.5 mg ($149) and 5.5 mg ($199) prices are all scheduled to end on December 31, 2026. A manufacturer's advertised price is not necessarily your final pharmacy price. Confirm the product, dose, pharmacy, insurance benefit and eligibility before you start.
This guide compares Wegovy (semaglutide), Zepbound (tirzepatide), Foundayo (orforglipron), Ozempic and Mounjaro, with the emphasis on what patients may actually pay. For the clinical comparison of these drugs, see our GLP-1 Hub and the Ozempic vs Wegovy vs Mounjaro vs Zepbound guide.

1. GLP-1 cost comparison: 2026 prices
These are advertised US manufacturer prices or savings-program prices, not guarantees of availability or checkout cost. They describe different payment situations: cash price, insurance-covered price, and list price.

| Medication | Form | Advertised self-pay price | If your commercial plan covers it | Key qualification |
|---|---|---|---|---|
| Wegovy | Daily tablet | $149 (1.5 mg), $199 (4 mg), $299 (9 mg or 25 mg) per month | As little as $25 (up to $100/month savings) | Price depends on dose; one month = 30 tablets. |
| Wegovy | Weekly pen | $199 for the first two fills (new patients, 0.25 mg and 0.5 mg, through Dec 31, 2026); then $349 per month | As little as $25 (up to $100/month savings) | One month = 4 pens. Introductory offer is for new patients only. |
| Wegovy HD (7.2 mg) | Weekly pen | $399 per month | As little as $25 (up to $100/month savings) | Novo Nordisk may change or cancel the program at any time. |
| Zepbound KwikPen | Weekly pen | $299 (2.5 mg), $399 (5 mg), $449 (7.5 mg to 15 mg) | KwikPen card applies only if your plan does not cover the KwikPen | One month = 28 days. $449 tiers need a refill within 45 days; offer ends Dec 31, 2026. |
| Zepbound single-dose pen | Weekly pen | As low as $499 per month if your plan does not cover it (card) | As little as $25 for up to a 3-month fill (card caps apply) | Card ends Dec 31, 2026. Not covered by the Medicare Bridge. |
| Foundayo (orforglipron) | Daily tablet | $149 to $299 per month depending on dose (regular prices up to $349) | As little as $25 (up to $100/month savings) | One month = 30 days. Some doses have temporary or refill-dependent prices. |
Why different websites show different prices: a product can have a wholesale list price (Novo Nordisk lists Wegovy tablets and pens at $1,349.02 per package), an ordinary cash price, a time-limited manufacturer offer and a separate price for insured patients. Those figures describe different situations. These products are not interchangeable: they differ in active ingredient, dose scale, clinical evidence, indications and safety considerations.
2. GLP-1 self-pay prices: what you need to know
Self-pay means paying outside your health insurance's prescription benefit. It can help when a plan excludes weight-management drugs, but the purchase ordinarily does not count toward your deductible or out-of-pocket maximum.
Wegovy self-pay pricing (NovoCare Pharmacy)
- Wegovy pill: $149 per month for 1.5 mg, $199 for 4 mg, and $299 for 9 mg or 25 mg.
- Wegovy injection, new-patient offer: $199 per monthly fill for the 0.25 mg and 0.5 mg doses, for two fills, through December 31, 2026.
- After the offer: $349 per month for 0.25 mg, 0.5 mg, 1 mg, 1.7 mg or 2.4 mg pens.
- Wegovy HD 7.2 mg injection: $399 per month.
Eligibility and restrictions apply, and Novo Nordisk reserves the right to modify or cancel the program at any time. The introductory price is not a promise of the same price for later doses or refills. Check the current NovoCare price list before filling.
Zepbound self-pay pricing (LillyDirect and other pharmacies)
| KwikPen dose | Self-pay card price | Regular price without the offer | Condition |
|---|---|---|---|
| 2.5 mg | $299 | $299 | Starting dose; not an approved maintenance dose. |
| 5 mg | $399 | $399 | Prescription and eligibility required. |
| 7.5 mg | $449 | $499 | Refill within 45 days of receiving the previous fill. |
| 10 mg | $449 | $699 | Refill within 45 days. |
| 12.5 mg | $449 | $699 | Refill within 45 days. |
| 15 mg | $449 | $699 | Refill within 45 days. |
One Zepbound month is defined as 28 days (one KwikPen). The self-pay card is for US or Puerto Rico residents aged 18 or older with a valid prescription, may be used for up to 11 fills per calendar year, and is scheduled to expire on December 31, 2026. It is separate from insurance: you may not seek reimbursement from an insurer or count the spend toward a deductible. If you refilled on time but the pharmacy could not verify it at checkout, Lilly allows a post-transaction reimbursement claim submitted by March 31, 2027. Lilly also advertises self-pay options for Zepbound vials starting at $299; confirm the dose-specific vial price on the official Zepbound savings page.
Foundayo self-pay pricing
Foundayo (orforglipron) is a once-daily oral GLP-1 receptor agonist approved in the United States in April 2026 for chronic weight management in eligible adults. Lilly separates regular prices from temporary offers:
Swipe the table sideways on mobile.
| Foundayo dose | Regular monthly price | Current offer |
|---|---|---|
| 0.8 mg | $149 | $149 |
| 2.5 mg | $199 | $149 through December 31, 2026 |
| 5.5 mg | $299 | $199 through December 31, 2026 |
| 9 mg | $299 | $299 |
| 14.5 mg | $349 | $299 with a refill within 45 days |
| 17.2 mg | $349 | $299 with a refill within 45 days |
One Foundayo month is defined as 30 days, and the cards allow up to 10 fills per calendar year. Additional taxes or fees may apply. See Foundayo's official pricing and coverage terms.
What about Ozempic and Mounjaro?
Ozempic (semaglutide) and Mounjaro (tirzepatide) are the diabetes-labeled brands; Wegovy and Zepbound are the matching brands approved for weight management. Do not compare brands on sticker price alone. Coverage may depend on whether the prescription is for type 2 diabetes or weight management, and a prescription for one brand does not qualify you for another brand's savings program.
3. Oral GLP-1 pills: are they cheaper than injections?
Oral options for chronic weight management in 2026 are the Wegovy tablet (oral semaglutide) and Foundayo (orforglipron). Both broaden choice, but their price structures and dosing rules differ.
- Wegovy tablet, $149 to $299 per month: taken once daily. Follow the label's instructions on taking it with water and waiting before eating or drinking.
- Foundayo, $149 to $349 per month at regular prices: taken once daily without the food and water timing restrictions of oral semaglutide. Certain doses carry temporary or refill-dependent discounts.
Does a pill automatically cost less? No. A low entry price may not reflect the maintenance dose, treatment duration, insurance benefit or the date an offer expires. Compare the cost of the dose you are likely to stay on, not only the starting dose. Also distinguish oral Wegovy from Rybelsus: Rybelsus is an oral semaglutide brand approved for type 2 diabetes and is not an interchangeable weight-loss substitute for Wegovy tablets.
Clinical note: price is only one factor. Expected benefit, contraindications, tolerability, other conditions and your clinician's assessment matter. Do not combine GLP-1 medicines unless a qualified prescriber directs it. See the semaglutide vs tirzepatide vs orforglipron comparison for efficacy and safety.
4. Does health insurance cover GLP-1 medications in 2026?
Sometimes, and coverage for type 2 diabetes and coverage for obesity are separate questions. A plan may cover Ozempic or Mounjaro for diabetes while excluding Wegovy or Zepbound for weight management, or it may cover an obesity drug but restrict the product, dose, pharmacy or patient criteria.
Employer and private insurance
The KFF 2025 Employer Health Benefits Survey found that 19% of firms with 200 or more employees that offer health benefits covered GLP-1 medicines when used primarily for weight loss, rising to 43% among firms with 5,000 or more employees. Among firms that covered them, 34% required a meeting with a dietitian, case manager or therapist, or participation in a lifestyle program. These are employer-level figures, not the share of all individual policies, and news reports in 2026 describe some employers narrowing or ending weight-loss GLP-1 coverage. Do not assume your plan follows a national average. Source: KFF, 2025 Employer Health Benefits Survey.
Formulary change to check: CVS Caremark removed its new-to-market block on Foundayo on June 1, 2026 and returned Zepbound to select formularies on October 1, 2026, according to Lilly and press reports. Plan sponsors (your employer or union) can still opt out of weight-loss GLP-1 coverage, so call your plan rather than relying on the PBM name alone.
Manufacturer savings cards for insured patients
If your commercial plan does cover the drug, each manufacturer offers a savings card that can lower your copay. These cards are capped, exclude government beneficiaries and are not insurance.
Swipe the table sideways on mobile.
| Product | Pay as little as | Savings caps | Other limits |
|---|---|---|---|
| Wegovy (pill or pen) | $25 per month | Up to $100 per month | Government beneficiaries excluded; Novo Nordisk may change terms. |
| Zepbound single-dose pen | $25 for a 1-, 2- or 3-month fill | Up to $100 per month; $1,300 per year | Up to 13 fills per year; ends Dec 31, 2026. |
| Foundayo | $25 for a 1-, 2- or 3-month fill | Up to $100 per month; $1,000 per year | Up to 10 fills per year; ends Dec 31, 2026. |
If your plan's copay or coinsurance is higher than $25 plus the monthly cap, you pay the difference. Cards also cannot be combined with other discounts and are not valid if your plan routes you through an alternate funding program.
What insurance restrictions should you expect?
- Prior authorization: your clinician may need to submit BMI, related conditions and treatment history.
- Eligibility criteria: the product's indication, BMI thresholds, weight-related conditions or a diabetes diagnosis.
- Step therapy: other approaches or drugs may need to be tried first, where the policy allows.
- Formulary restrictions: a plan may prefer one product or dose.
- Continuation requirements: renewal may depend on response, adherence or updated documentation.
- Employer exclusions: a plan can exclude anti-obesity medicines even if it covers other GLP-1 drugs.
Coverage also differs between self-funded employer plans, fully insured employer plans and individual policies. "Covered" on a manufacturer's website does not establish that your plan will pay.
How to verify coverage before you start
- Ask your insurer or pharmacy benefit manager whether the exact drug and formulation are covered for your indication.
- Request the formulary, prior-authorization criteria, preferred alternatives and exclusions.
- Ask for your estimated cost at the starting dose and the expected maintenance dose.
- Confirm whether a manufacturer savings card can be used with your plan.
- Check whether authorization must be renewed and what happens if you change employers or plans.
Get answers in writing when possible. A pharmacy-benefit representative can explain the prescription benefit; your clinician assesses medical eligibility and submits documentation.
5. Medicare GLP-1 coverage in 2026: the $50 monthly Bridge
From July 1, 2026 through December 31, 2027, eligible Medicare beneficiaries with Part D drug coverage can get selected GLP-1 medicines for weight management through the temporary Medicare GLP-1 Bridge for a $50 monthly copayment. It is available nationwide and works with standalone drug plans, Medicare Advantage plans with drug coverage, special needs plans, employer or union group waiver plans and the LI NET program.
Which products are included?
- Wegovy: tablet or injection.
- Foundayo: tablet.
- Zepbound: KwikPen only. Single-dose pens and single-dose vials are not covered by the Bridge, and pen needles are sold separately.
Who may qualify?
You must be 18 or older and meet one of these when you start therapy:
- BMI of 35 or higher; or
- BMI of 30 or higher with heart failure with preserved ejection fraction, uncontrolled hypertension, or chronic kidney disease stage 3a or higher; or
- BMI of 27 or higher with prediabetes, a previous heart attack or stroke, or symptomatic peripheral artery disease.
You are not eligible if you already get GLP-1 drugs covered through your Part D plan, or if you have type 2 diabetes, moderate-to-severe sleep apnea or fatty liver disease (your regular Part D plan may cover GLP-1 treatment for those conditions instead).
How it works and what the $50 does not do
- Your prescriber sends the prescription and, when requested, completes a prior authorization, certifying that the drug is used with a diet-and-exercise lifestyle program. You receive a letter when coverage is approved, and the authorization is valid, including refills and dose changes, through December 31, 2027, unless you switch GLP-1 drugs.
- The program runs outside the normal Part D benefit. The copay does not count toward your Part D deductible or out-of-pocket limit, cannot be lowered by Extra Help, and cannot be spread out with the Medicare Prescription Payment Plan.
- One month means a 28- or 30-day supply, depending on the drug.
- Manufacturer $25 savings cards are not available to Medicare enrollees.
The Bridge is not universal Medicare coverage for every GLP-1 medicine or every person with obesity. Medicare may cover GLP-1 drugs under regular Part D for other FDA-approved indications, subject to plan rules. Do not use the BMI list above as a substitute for the full eligibility review.
Official information: Medicare.gov: Weight-loss drugs, CMS: Medicare GLP-1 Bridge, or 1-800-MEDICARE (1-800-633-4227).
6. Medicaid, ACA Marketplace and other coverage
Medicaid
Coverage of obesity medications varies by state, and reports indicate some states scaled back coverage in 2026. Do not assume a drug covered by a commercial insurer is covered by Medicaid. Check your state's current formulary and prior-authorization rules with your prescriber or the state Medicaid agency. Manufacturer savings cards exclude Medicaid enrollees.
ACA Marketplace and individual plans
Coverage depends on the policy. Diabetes drug coverage does not necessarily mean branded weight-management drug coverage. Read the formulary and plan documents, not only the summary of benefits.
Other government programs
VA, TRICARE and other government-funded programs have their own rules and are excluded from the commercial savings cards. Lilly's self-pay card for the Zepbound KwikPen and Foundayo is a cash option that can be considered separately, because it operates outside insurance.
What savings programs can and cannot do
- Some offers require commercial insurance that covers the drug; others are cash-pay options for people without coverage.
- Offers can be limited by dose, number of fills, refill timing and expiration date.
- A savings card is not insurance and does not pay for consultations, labs or other medical services.
7. How to reduce your GLP-1 cost
Price the whole treatment, not the first fill
A low starting-dose offer may not reflect the price after dose escalation. Ask for the price of the dose you are likely to maintain.
Protect your refill timing
The $449 Zepbound tiers and the $299 Foundayo high-dose price both depend on refilling within 45 days of receiving the previous fill. Missing that window can raise your price to the regular rate.
Use official price and coverage tools
- NovoCare: Wegovy prices and savings
- Lilly: Zepbound savings and coverage
- Lilly: Foundayo savings and coverage
- TrumpRx.gov: a federal price-comparison site referenced by Medicare.gov. You cannot buy drugs directly there, and discount-site purchases are not creditable coverage, so they do not count toward a Medicare deductible or out-of-pocket maximum.
Confirm the program terms on the day you fill the prescription, because several 2026 offers are explicitly time-limited.
Compare legitimate pharmacies
For a valid prescription, ask whether a quote reflects insurance, an eligible manufacturer offer or a cash price, and confirm formulation, quantity, delivery charges and fees. For telehealth options, see our guide to comparing telehealth peptide and GLP-1 providers.
Ask about alternatives and assistance
If branded treatment is unaffordable, discuss clinically appropriate alternatives with your clinician. The cheapest medicine is not necessarily the best option for every patient.
Avoid unverified products
Be cautious of sites selling "research peptides", counterfeit pens or unverified compounded products at unusually low prices. Products outside authorized supply channels may not offer the same assurance of identity, quality, handling or dose. Do not replace an approved prescription or change your dose because another online product looks cheaper.
8. Hidden costs and long-term budgeting
- Clinical care: consultations, follow-up visits and laboratory tests may be billed separately.
- Supplies: pen needles for the Zepbound KwikPen are sold separately under the Medicare Bridge.
- Administration: prior authorization, renewals and documentation can affect timing.
- Nutrition and activity support: dietitian visits or structured programs may add cost. See our guide to preventing muscle loss on GLP-1 drugs.
- Fill caps and expiry: savings cards limit fills per year and several end on December 31, 2026.
- Long-term treatment: plan for dose changes, plan changes and the chance that a promotion expires.
How much could a year cost?
Multiply a monthly price by 12 only if that price truly applies to all 12 fills.
Swipe the table sideways on mobile.
| Monthly price | 12-month total | Where you may see it |
|---|---|---|
| $50 | $600 | Medicare GLP-1 Bridge (eligible beneficiaries) |
| $149 | $1,788 | Entry-dose pill prices |
| $299 | $3,588 | Higher pill doses; Zepbound 2.5 mg |
| $349 | $4,188 | Wegovy pen after the intro offer |
| $449 | $5,388 | Zepbound KwikPen 7.5 mg to 15 mg |
These are arithmetic examples, not predictions. They exclude consultations and supplies, and they must not be projected beyond an offer's expiration date.
9. GLP-1 cost and coverage decision checklist
Before you start or renew, answer these with your prescriber and insurer:
- Which exact drug, formulation and dose is prescribed?
- What is the real price for the first fill and for the expected maintenance dose?
- Does my plan cover this product for my indication?
- Is prior authorization required, and how is renewal handled?
- Am I eligible for the manufacturer's current savings program or card?
- When does the offer expire, and what are the refill-timing conditions?
- What will six and twelve months cost if prices change?
- What is my plan if coverage is denied or my insurance changes?
For Medicare beneficiaries, add: do I qualify for the Medicare GLP-1 Bridge, and is my formulation (for Zepbound, the KwikPen) included?
10. Using this page with AI assistants
AI assistants can help you organize questions for your insurer, but they cannot see your plan or confirm today's prices. Do not paste member IDs, birth dates or other identifiers. Verify any price an assistant gives you on the official pages linked above.
Swipe the table sideways on mobile.
| Assistant | Good use for this topic | Example prompt |
|---|---|---|
| Claude | Reading a formulary or denial letter (with identifiers removed) and drafting questions or an appeal outline | "Here is the weight-management section of my plan's formulary. List what it says about prior authorization and step therapy for Wegovy, Zepbound and Foundayo." |
| ChatGPT | Building a 12-month budget with assumptions you supply | "Build a 12-month cost table using these prices and tell me what changes if the offer ends December 31, 2026." |
| Gemini | Comparing pharmacy and plan documents saved in your own workspace | "Summarize my plan's pharmacy benefit and flag any mention of anti-obesity medications." |
| Perplexity | Finding the current official page and its update date | "What is the current self-pay price of the Zepbound KwikPen 7.5 mg on Lilly's official site, and when does the offer end?" |
Source grading for this page
Primary, current: manufacturer pricing and terms (NovoCare, Lilly) and federal program pages (Medicare.gov, CMS), all checked on October 18, 2026. Survey data: the KFF employer survey describes employer firms, not individual plans. News reports: used only for the CVS Caremark formulary change and employer trends and treated as less authoritative than the primary pages. CEBM evidence tiers apply to clinical efficacy and safety claims, which are graded in the GLP-1 Hub, not to price data.
11. Frequently asked questions
How much do GLP-1 drugs cost per month in 2026?
It depends on the drug, dose, insurance and offer. Advertised US self-pay prices run from $149 per month (Wegovy pill 1.5 mg, Foundayo 0.8 mg) up to $449 per month for most Zepbound KwikPen maintenance doses. Eligible commercially insured patients whose plan covers the drug may pay as little as $25 per month with a savings card, and eligible Medicare Part D patients pay $50 per month through the Medicare GLP-1 Bridge.
Which GLP-1 pill is cheapest in 2026?
Wegovy tablets (1.5 mg) and Foundayo (0.8 mg and, through December 31, 2026, 2.5 mg) both start at $149 per month self-pay. Compare the dose you will actually take, because higher doses cost $299 to $349 per month at regular prices.
Does insurance cover Wegovy or Zepbound?
Some plans do, many do not. Coverage depends on your employer or plan, the exact product, your diagnosis and prior-authorization rules. Ask your plan about the exact product and dose before you start.
Can I get Zepbound for $299 per month?
Lilly's self-pay price of $299 per month applies to the 2.5 mg starting dose of the KwikPen. Higher doses cost $399 (5 mg) or $449 (7.5 mg to 15 mg, with a 45-day refill condition). The 2.5 mg dose is not an approved maintenance dose.
Can Medicare cover GLP-1 medication for weight loss in 2026?
Eligible Medicare Part D beneficiaries can get Wegovy (tablet or injection), Foundayo or the Zepbound KwikPen for a $50 monthly copay through the Medicare GLP-1 Bridge, from July 1, 2026 through December 31, 2027. BMI and medical-condition criteria, prior authorization and a lifestyle-program certification apply.
Can I use a manufacturer savings card with Medicare or Medicaid?
No. The $25 commercial savings cards exclude government beneficiaries, including Medicare, Medicaid, VA and TRICARE enrollees.
Are Ozempic and Wegovy the same medication?
Both contain semaglutide, but they are separate brands with different FDA-approved uses, doses and coverage rules. A prescription or discount for one does not automatically apply to the other.
Do self-pay purchases count toward my deductible?
Usually not. Cash purchases made outside your insurance generally do not count toward your deductible or out-of-pocket maximum. Confirm with your insurer before paying cash.
What happens to GLP-1 offers on December 31, 2026?
Several offers are scheduled to end on that date, including the Wegovy new-patient introductory offer, the Zepbound and Foundayo savings cards and the temporary Foundayo 2.5 mg and 5.5 mg price cuts. Recheck official pages before budgeting for 2027.
